Working alongside the Social Work team, the Behavioral Health Specialist serves as part of a clinical IDT to represent and advocate for each participant’s behavioral health needs. The Behavioral Health Specialist will play a critical role in overseeing a caseload of participants for all their therapy needs, as well as ensuring proper documentation.

This role is different because the Behavioral Health Specialist at WelbeHealth:

  • Works as part of a centralized, close-knit Behavioral Health team supporting participants, creating consistency and collaboration across markets
  • Partners with Social Work, Psychiatry, and other members of the IDT to deliver coordinated, participant-centered care
  • Is trained using the Collaborative Care Management approach, strengthening communication, teamwork, and participant outcomes

On the day-to-day, you will:

  • Conduct behavioral health assessments to determine participants’ behavioral health care needs, preferences, and goals
  • Actively participate in IDT meetings to develop discipline-specific goals and interventions for each participant
  • Deliver and document behavioral health interventions as agreed upon in participants’ care plans, including but not limited to symptom management, counseling, crisis intervention, arranging necessary resources and services, assisting with care transitions, providing individual and group counseling, and care management
  • Facilitate individual and group therapies such as art and music therapy, psychosocial drama, counseling, and discussion that restore or expand participants’ social roles
  • Provide training on behavioral health symptoms and interventions to WelbeHealth staff

Job requirements include:

  • Master’s degree in behavioral health or doctorate degree in psychology required
  • Licensed by state Board of Behavioral Sciences as an LCSW or licensed by the state Board of Psychologists as a Psychologist
  • Minimum of five (5) years of behavioral health experience conducting therapy, completing behavioral health assessments, and managing care planning needs
  • Minimum of two (2) years of experience with frail or elderly populations
