The Community Health Worker I provides direct center and field-based support to PACE participants by addressing Social Determinants of Health (SDoH) needs and reducing barriers to accessing essential services. This role focuses on assisting participants with connections to community resources, supporting application completion for community programs, and ensuring participants are able to access services that support health, safety, and stability.

On the day-to-day, you will:
- Provide direct participant support to address SDoH needs through home visits, outreach, and in-person assistance focused on accessing essential services and resources
- Connect participants to established community resources (e.g., housing supports, food insecurity resources, utility assistance, caregiver support, transportation resources, etc.) and follow up to support access, eligibility verification, and utilization
- Assist participants with completing and submitting applications for identified community resources and community-based programs
- Support Social Workers in discharge planning by supplying applicable community resource information during PACE disenrollment
- Support participants with complex needs through ongoing engagement, resource connection support, and barrier resolution that directly impacts health outcomes and quality of life
- Collaborate with Social Workers, ECAs, PELs, and the broader IDT to reinforce participant-centered care plans and ensure coordinated resource support
- Assist with completing grievances, appeals, incident reports, APS reports, and other quality or compliance-related requirements as needed
Job requirements include:
- High school diploma or equivalency required
- Associate’s degree in social work, human services, social science, or relevant field preferred
- Reliable means of transportation
- Strong knowledge of community-based services and resource navigation
- Experience working with older adults or vulnerable populations is a plus
- Ability to work independently with minimal supervision
