Under the direction of the Executive Director, Clinical Nurse Manager or Clinical Nurse Director, the WelbeHealth Assistant Clinical Nurse Manager II is accountable for organizing and directing the day-to-day RN Case Manager program and may also have oversight of the clinic staff, directing the day-to-day work at the clinic, and coordinating clinic flow to ensure effective WelbeHealth clinical services are provided, and quality of care standards are met.

On the day-to-day, you will:
- Provide leadership, oversight, aid, guidance, and performance management of RN’s, ensuring all have role clarity and the training required to perform their functions at the top of their license. May have oversight of RNs across multiple sites.
- Oversee the clinical needs in the center, to ensure chronic and acute needs of participants are addressed properly and liaise with the Interdisciplinary team on smooth transitions of care to meet post-discharge needs.
- May Provide leadership, oversight, aid, guidance, and performance management of LVN’s and MA roles, ensuring all have role clarity and the required training to perform the functions required of their role.
- Develop workforce plans and ensure appropriate staffing, while recognizing the diverse experience levels of staff members and the unique needs of the participant population.
- Effectively intercede with participants and their families and resolve concerns related to the delivery of health care services.
- Manage direct reports including hiring, training, supervising, and mentoring. May have direct reports across multiple sites.
- Follow WelbeHealth policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
Job requirements include:
- Graduate of an Accredited School of Nursing with unencumbered Registered Nurse (RN) License with which you practice required; Bachelor’s (BSN) degree preferredExperience Requirement.
- One (1) year of supervisory experience with demonstrated aptitude to mentor and develop team members preferred.
- Minimum of 2 years of case management, care coordination or related specialty.
- Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
- Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
- Highly motivated, inspirational leader who can engage, support and remove barriers for the team to ensure the team consistently meet established goals.

Salary/Wage base range for this role is $132,181- $174,478 / year + Bonus. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
