The Role:

Working in small teams (Care Pods) with other highly capable and passionate clinicians, the Psych ARNP will provide comprehensive psychiatric evaluations, diagnosis, and treatment to patients affected by dementia. This role involves collaborating with a multidisciplinary team to develop and implement individualized treatment plans that address patients' psychiatric needs and promote overall well-being.

Responsibilities:

  • Develop high-quality therapeutic alliances with patients and their caregivers, providing direct patient care for dementia issues as well as providing direct patient care for behavioral health issues
  • Guide patients and their families through the journey of dementia and late life mental illness
  • Provide BH expertise to the organization through development of policies & procedures, training and general guidance
  • Serve in a consultative role to all NPs and Care Pods with regard to more complex behavioral health patient care situations, participating in interdisciplinary team meetings, case conferences, and consultations to discuss patient care and treatment strategies
  • Provide ongoing cognitive, behavioral and psychopharmacological assessments and treatments
  • Prescribe and manage psychiatric medications, including monitoring medication efficacy and side effects
  • Coordinate patient care with primary care providers and specialists, nurses and home health aides, including handling more complex cases
  • Coordinate patient care with Rippl social workers and Care Navigators
  • Provide advanced care planning for both patients and families
  • Provide dementia care planning and chronic care management
  • Stay informed about current research, best practices, and evidence-based treatments in psychiatric care through continuing education and professional development activities
  • Adhere to legal and ethical standards of practice, including maintaining patient confidentiality and upholding professional boundaries

Qualifications:

  • Master's or Doctoral degree in Nursing with a specialization in psychiatric-mental health
  • An active licensure in TX or IL with unrestricted ability & willingness to license in other 50 states per business needs
  • Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP)
  • A deep passion for taking care of older patients and developing longitudinal care relationships with them and their families
  • Interest in learning a new skill set and pioneering a new workforce
  • Comfort working with interdisciplinary teams
  • Geriatric Medicine, Palliative Care or Chronic Care management experience preferred
  • 1-2+ years of experience managing older adult patients in outpatient, inpatient or long-term care settings (preferred but not required)
  • Strong clinical assessment and diagnostic skills, with the ability to formulate comprehensive treatment plans
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and their families
  • Proficiency in electronic health record (EHR) systems and other healthcare technology tools (Athena preferred)
  • Ability to work collaboratively as part of a multidisciplinary team and to function independently with minimal supervision
  • Commitment to professional growth and ongoing learning in the field of psychiatric-mental health nursing
  • Valid driver's license and proof of motor vehicle insurance

Pay Range Details

The pay range(s) below are provided in compliance with state specific laws. Pay ranges may be different in other locations. Exact compensation may vary based on skills, experience, and location.

Role: Range is $145,000 - $160,000 (DOE)
