About the Role
We are looking for an experienced patient care leader with deep expertise supervising leaders in high-growth behavioral health environments. You are familiar with our patient population: adolescents and transitional aged youth who have recently been discharged after a psychiatric hospitalization or are in foster care.
As Head of Patient Guides, your primary responsibility will be recruiting, onboarding, supporting, and managing a high-performing team of Directors of Patient Guides. You will ensure consistent, high-quality patient engagement and strong fidelity to the Flourish integrated care model as the organization scales rapidly.
You are an exceptional people manager who knows how to recruit strong leaders, onboard them effectively, support teams through change, and create accountability while maintaining a sustainable, supportive culture. You are a strategic leader in both remote and in-home patient care environments, and you can lead a scrappy hybrid department with excellence.
Role Responsibilities:

Productivity:
Oversee patient guide vertical for up to 10 bundles of carepods, which is up to 4,500 high acuity patients
Each bundle has up to 15 carepods, and the key metric is for mature carepods to have a patient panel size of 25-32+ engaged patients

People Management:
Recruit, onboard, and manage Directors of Patient Guides, each of whom oversee patient guides on a bundle of up to 15 carepods. This means that each of your direct reports has up to 15 direct reports themselves.
Build systems to support consistency, accountability, and strong clinical leadership across patient guide teams
Coach Directors of Patient Guides on clinical supervision, people management, productivity, and change management
Support leadership coverage planning during PTO, leaves, and turnover. Support leadership coverage planning during PTO, leaves and turnover, including taking on interim direct reports or carepod leadership tasks as needed

Clinical & Operational Excellence
Ensure consistent, high-quality patient care across all carepods
Maintain strong fidelity to the Flourish integrated care model at scale
Support Directors of Patient Guides in managing clinical escalations and complex patient care situations
Support Directors of Patient Guides in building and maintaining guide in-home visit expectations and protocols
Identify operational or clinical gaps and implement scalable improvements

Cross-Functional Collaboration
Partner closely with leadership across psychiatry, therapy, family guides, operations, recruiting, and people operations
Help ensure integrated carepods function effectively and consistently across bundles
Support organizational scaling initiatives and workforce planning

Change Management:
Startups have changes all the time, sometimes weekly, and this role is for a team leader who will be responsible for the change manage of their team
Build trust and motivation with team, owning the responsibility for strong culture and employee engagement through changes

Patient Care
A core leadership tenet at Flourish is leading by example. Therefore all clinical leaders provide direct patient care for 2-5 hours per week.
Role Requirements:
10+ years of relevant patient coaching related experience
10+ years of people management for full time direct reports
2+ years experience leading multi-state behavioral health teams
2+ years experience managing through layers of leadership
8+ years of startup experience
Able to work one evening per week
Must be 18 years or older
Job Type: Full-Time
Work location: Remote, with in-person event attendance required 2+ times per year
Salary: $150,000-$170,000
