About the role:
The Practice Coordinator will be responsible for assisting with providing seamless, high-quality care to patients enrolled in the Equip eating disorder program and administrative support to provider staff. The Practice Coordinator will be highly organized and motivated to complete complex tasks. The Practice Coordinator should be able to work closely and effectively with multiple members of the clinical care team to ensure appropriate communication and build rapport with outside clinical providers.

Responsibilities:

  • Works collaboratively to assign patients to appropriate treatment team and ensure all patient intakes are scheduled appropriately
  • Serves as a liaison between Admissions, provider teams, and patients ensuring a seamless transition from prospective patient to active patient
  • Function as the administrative coordinator for designated group of providers (varied disciplines) and provide end-to-end service for providers including but not limited to:
  • Complete tasks for clinical staff including data entry, faxing, scheduling, and collection of outside medical records
  • Provide expert customer service for patients who are onboarding at Equip and ensure a smooth entry into Equip treatment
  • Intentional, collaborative, and service-oriented communication with providers, patients, and outside referrants
  • Coordinate with the provider team to retrieve and compile clinical information related to ongoing authorizations, treatment summaries, outside provider updates, and patient requests (e.g. excusal letters, FMLA guidance, etc.)
  • Provide ongoing scheduling support to providers Coordinate discharge planning with the clinical team to ensure all patients are appropriately prepared for discharge appointments and follow up
  • Serve as a resource for an introduction to Equip programming for patients and provide ongoing support for patients who need engagement support
  • Manages out of office messages and planning for providers to ensure smooth experience for patients
  • Communications with current patients regarding closures, unexpected changes, or new options for schedules
  • Works with manager to adhere to templated schedules for provider scheduling Serve as a navigator for patients who need extra support finding community resources and managing treatment demands
  • Point of contact for providers regarding all administrative tasks including: patient pauses or transitions in care, outside records, family concerns/complaints, and reassignments.
  • Perform other duties as assigned.

Requirements:

  • Experience in front desk, reception, and/or clinic administration
  • 4+ years of experience with working with medical and clinical providers
  • Highly organized, skilled administrator
  • Strong communicator with a high level of confidentiality
  • Skilled at navigating electronic medical records
  • Ability and willingness to pivot to other potential tasks
  • Enthusiastic, strong team player a must
  • Preference is a candidate in PST
