A bit more about this role:

This is an exciting opportunity for a compassionate and skilled Advanced Practice Provider (APRN or PA) to provide virtual primary care to a panel of patients through our innovative telehealth platform. As a Primary Care Advanced Practice Provider (Longitudinal Chronic Disease Management), you’ll manage that panel of patients on their primary care journey with Devoted Medical. You’ll conduct comprehensive clinical assessments, develop plans for chronic condition management, and work on preventative and proactive care. You’ll collaborate closely with specialists, pharmacists, nurses, care coordinators, caregivers, and families to deliver coordinated, member-centered care.

You will also provide that panel with ongoing management of their new and chronic conditions, preventive screenings, and medication management, all through a virtual care model. You'll work collaboratively with a multidisciplinary team to coordinate in-person services such as lab work, imaging, and specialist referrals as needed. You’ll partner with our Physician team on more complex patients.

Responsibilities will include:

  • Deliver longitudinal virtual primary care visits via telehealth to a panel of patients.

  • Be the anchor to an interdisciplinary primary care team, leveraging urgent care, specialty, digital teams to deliver access and quality care

  • Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease, etc.) and develop individualized care plans to optimize patient health.

  • Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.

  • Manage medication prescriptions including controlled substances, titrations, and refills, ensuring patient adherence and safety.

  • Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health.

  • Identify patients needing referrals, or home-based services, and facilitate appropriate coordination with specialists or community resources.

  • Utilize and provide feedback on our electronic health platform, contributing to its continuous improvement to enhance patient and provider experience.

  • Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care.

  • Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management.

Desired skills and experience:

  • Active Advanced Practice Provider (APRN or PA) license in good standing in either AL, AZ, AR, CO, DE, FL, GA, HI, IL, IN,IA, KS, KY, LA, MS, MO, NE, NM, NC, OH, OK, OR, PA, SC, TN, TX, UT, WA or VA (must be willing to obtain additional licenses and a DEA certification within the first 90 days — support and reimbursement provided)

  • Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.

  • Minimum 4 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.

  • Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.

  • Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.

  • Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).

  • Active BLS certification at time of hire.

  • Shift times may vary in order to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone. Flexibility is required. Additionally, weekend shifts may be required in the future based on evolving patient care needs.

Salary Range: $120K-155K base range plus performance based bonus paid out quarterly or annually ($10K-$20K) for a total comp range of $130K-170K

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
